Your physician has referred you to our office for a neurological evaluation and/or diagnostic study. Following your visit, a comprehensive report will be provided to your physician including any findings and recommendations.

As a new patient to our office, please take the time and register in our Patient Portal.

If you cannot access the portal, please arrive 30 minutes prior to your appointment time to complete any necessary paperwork. Also, please complete and bring with you the attached forms as well as all medical records related to the reason for your visit including CD’s/Films of MRI’s, CT scans and X-rays. A complete list and/or bottles of ALL medications you are currently taking including vitamins/supplements, etc. We have provided a map on the location area of our web site for your convenience.

Our appointment schedule is made with great care to provide you with an on time, satisfying visit. Your assistance is greatly appreciated. We will confirm your appointment in the days leading up to it. Please respond as requested at that time. Appointments that are not confirmed may be canceled to make room for patients on our wait–list. Confirmed appointments that are not kept or canceled with at least 24 hours notice, will be subject to a $50 fee.

Kindly be prepared to pay your share of any charges at the time of your visit including co-payments, co-insurance or deductibles. We will assist your whenever possible in the filing of any insurance paperwork. Our office accepts Visa, Mastercard, American Express, Discover, personal checks, debit cards and cash payments. There is a $25 fee for any returned checks.
